早速ですが、昨日、急逝された岸本知事
報道では、万博開幕式欠席、開幕日も和歌祭のところ以外は、代理の方にお願いしたと聞こえてます
さらに、倒れられる前に腰痛があったとも言われています
岸本知事は特にエネルギッシュな責任感のある方でした
今後、和歌山県含めて知事さんなどの要職の方は、検査体制のさらなる充実と、もっと大事なことは、休息つまり休みをしっかり設けてあげて頂きたいと思います
もっとも、検査体制や休息体制は、しっかりされていると思いますが、より精度を高めてより休息を与えてあげてほしいという一県民からの意見となります
逆に言えば、行政が、知事に休息を与えやすいような、都道府県住民の知事の行事参加要請などへの理解も不可欠かと思います
さて、今回に関してでなく
腰痛で当院にご来院された患者様には
やはり極、稀にレッドフラッグつまり
これは、高度医療機関への受診を促すことに徹します
そこには、専門外でも、それ相応の知識と最後はおかしいと思う直感で即、紹介状となります
過去に当院で高度医療機関に受診されたらどうですか？と
かなりセンシィティブな問題なので穏便にオブラートに包んで、診断は専門医からお伝え頂くというスタイル
その中で
腰部なら
女性では、子宮などの疾患
男性では、悪性腫瘍の転移(脊髄転移)
頸部なら
脳梗塞(これはかなり多い)
心筋梗塞
上記疾患の紹介された方はほぼ、紹介先でも同じ診断をされました
腫瘍転移は別にして、早期発見で
患者様には喜ばれました
それは当院としてもほっとします
話は元に戻すと、腰痛に関して、他に気になる症状があるとすれば腎盂腎炎を疑うかと思います
なぜなら、急性腎盂腎炎ならそれ相応の症状があるわけで、かなり専門外なので詳細は伝えかねますが、風邪だけなら腰痛がメインとしてご来院されにくいと推測します
しかしながら、当院に来られる患者様は当然それらを全てわかった上で評価される(患者様から見たら診断される)と思われていると思います。
そのためにということではなく、
的確な評価、処置、施術は、不可欠なので、施術者に引き出しが多くないと閃きません。
特にレッドフラッグを見落とすことは、その方の生命にかかることですので、専門外だったとか整骨院であろうが、許されません

I would like to bring to your attention the sudden death of Governor Kishimoto yesterday.
According to news reports, he was absent from the opening ceremony of the Expo, and even on the opening day, except for the Waka Festival, he asked his deputy to attend the event.
It is also said that he had been suffering from back pain before he collapsed.
Governor Kishimoto was a particularly energetic and responsible person.
From now on, I hope that the governors and other important officials, including those of Wakayama Prefecture, will further improve the inspection system and, more importantly, provide for rest, or in other words, take a good rest.
I believe that the inspection and rest systems are well established, but I would like to express my opinion as a citizen of Wakayama Prefecture that the inspection system should be made more accurate and that more rest should be given to the residents of Wakayama Prefecture.
Conversely, I think it is essential for the administration to understand the events of the residents of the prefecture so that they can easily give the governor a rest.

I’m sure there are red flags, which are very rare.
This is why we always encourage patients to seek medical attention at advanced medical institutions.
Even if they are not specialists, we will immediately send them a letter of referral based on our knowledge and our intuition that there is something wrong.

Cerebral infarction (this is quite common)
Myocardial infarction
Almost all of the patients referred for the above diseases were given the same diagnosis at the referring hospital.

Back to the subject at hand, if there are other symptoms of back pain that concern you, I would suspect pyelonephritis.
Because acute pyelonephritis has its own symptoms, which I can’t go into detail about because I am not a specialist, but I would guess that if you only had a cold, you would be less likely to come to our clinic with back pain as your main symptom.
However, I believe that patients who come to our clinic are naturally expected to be evaluated (diagnosed from the patient’s point of view) after knowing all of the above.

In particular, missing a red flag is unacceptable, even if the patient’s life depends on it, even if the patient is not a specialist or is in an osteopathic clinic.
